How long do I have to notify you if I have an accident?
ANS:We must receive notification of an accident within 90 days after the accident in order for your treatment to be covered.

What information do I need to submit when filing for reimbursement?
ANS:You must send us an accident report and a completed claim form that has been signed by a school official.

Who do I tell the doctor/hospital who the insurance company is?
ANS:The insurance company is not the same for all groups, so please check your claim form for the name of your insurance company. However, all claims need to be filed with your plan administrator, which is Fringe Benefit Coordinators.

What does the policy cover?
ANS:It covers accidents that occur while you are participating in a class activity.
