How long do I have to notify you if I have an accident?
ANS:We must receive notification of an accident within 90 days after the accident in order for your treatment to be covered.

What information do I need to submit when filing for reimbursement?
ANS:You must submit a fully completed notice of claim(claim form) that has been signed by the trainer/school official and your parent or legal guardian. Incomplete claim forms will delay payment of your doctor/hospital charges.

Who do I tell the doctor/hospital who the insurance company is?
ANS:The insurance company is not the same for all groups, so please check your claim form for the name of your insurance company. However, all claims need to be filed with your plan administrator, which is Fringe Benefit Coordinators.

What does the policy cover?
ANS:It covers accidents that occur while you are participating in athletic/UIL activities

If my child has other insurance through my employer, how do I file my claims?
ANS:The insurance through the school is secondary to all other coverage. Submit, or have the provider submit, all your bills with your insurance first. After they have paid or fully denied the charges, send a copy of the bill(s) along with the Explanation of Benefits from your primary insurance carrier to Fringe Benefit Coordinators.
