Can you advise me what I should do if I determine I was overcharged for a treatment or service that was provided?
ANS: Please provide the date of service, provider name, and any other pertinent information you may have. We will then assess the claim, work with the provider on the adjustment amount, and advise you of the outcome.

Do I need to confirm my insurance coverage prior to scheduling treatments or having services performed?

Can you advise me if a deductible or coinsurance amount will be due prior to a service being provided?
ANS: Yes, call our servicing agency, F.B.C., at 1-800-654-1452. If you are in Gainesville, call 377-1239.

Does my policy cover treatment or services when they are provided out-of-state or out of the country?
ANS: Yes. However, bills from out of the country must be submitted in U.S. currency. UNUM’s Assist America program is also useful to help you navigate the maze of health care whenever you are more than 100 miles from home. Their phone number is on your I.D. card.

Can I obtain a copy of a claim submitted by a provider, especially if my treatment or service provided was not routine service?
ANS: Please contact the doctor or facility for a copy of the claim.

Can I change my provider anytime I want?
ANS: Yes.

How do I get a new I.D. card?
ANS: First, notify your benefits/payroll coordinator at your School District office. She/he will notify the servicing agency and the I.D. card mailed to the school.

How can I find out the status of my claim?

How can I get help with a personal issue?
ANS: Most school districts participate in the LifeBalance program. LifeBalance is a one-stop resource that offers consultation, information, and personalized community referrals available 24 hours a day, 7 days a week for you and your family members.
